Grayson County Health Department’s Community Health Worker (CHW) serves as a liaison between multiple services in our community and our community members.  The CHW is responsible for helping our clients and their families navigate and access community services and resources.   The CHW provides social support and informal counseling while advocating for individuals and community health needs.  Our CHW also assists clients with prescription medication fees through the Kentucky Prescription Assistance Program (KPAP).
